Support Coordination and Specialist support coordination
Support coordination helps you to achieve your NDIS plan and goals by connecting you with informal, mainstream, community, and funded groups.
Midel group will deliver services within the funding available on the individual NDIS plan by considering your specific needs and delivering supports around any challenges you may face.
Midel group will also teach skills needed to understand your plan, maintain positive relationship and develop confidence and ability to control your own future.
Our support coordination team are incredibly passionate about supporting people to implement their individual tailored NDIS plan by:
- Providing people with the skills needed to understand their plan, finding services and support, and helping people to identify and connect with service providers.
- Providing a good mix of support by working collaboratively to explore local community and mainstream services as needed for the individual
- Facilitating the development of future goals for their next plan and by communicating with support circle (family / loved ones) about alternative options as the year progresses.
- Support you in keeping track of your plan to ensure NDIS funds are used effectively and Providing support in navigating unforeseen changes/challenges.
- Coordinating service agreement, liaising with service providers, and acting as NDIS contact person to discuss/ provide assistance to solve any issues that may arise.
- Providing NDIA progress report and support in preparing / coordinating for plan review meetings.
Social Work / Family Support
Midel group social workers work with Kids, young people, and their families to provide support, connecting them with communities and accessing the services needed.
Social workers provide guidance, information, and resources to help your whole family to better cope and flourish together. Social workers can also provide.
- Therapy for goals around improving social connection and self-esteem.
- Emotional regulation skills including assistance to manage thought, feelings, and emotions.
- Support with understanding your plan, coordination and connecting you with relevant service providers.
- Support around building positive relationships during times of crisis and challenges.
- Family support services that involve counselling, case management, programs/ events to help family members build skills required for them to provide care for kids living with disability.
- Information and referral to other NDIS services
- Advocacy services for NDIS participants and families experiencing social disadvantage and difficulties with education, employment, housing, health care and other basic rights.
We are able to support you even when social workers is not listed specifically in your plan. Social work can be billed through the capacity building budget (Improved daily living skills) so if your child has Capacity building budget on their NDIS plan, social work can be accessed.
Occupational Therapy
Occupational therapists can assist individuals and families who experiences difficulties in participating in the activities of their daily life. Activities such as personal care, sleep, emotional regulation, vision, fine and gross motor skills, social skills (playing with others), independent living skills, and engaging at school or communities.
Occupational Therapist works with individuals to facilitate therapy and strategies needed to participate in functional activities.
Occupational Therapist will work closely with individuals and families to develop strategies that best meets the individual’s goals in order to promote independence and skills that can support individual to participate actively and independently in activities of their life.
Speech Pathology
Speech pathology therapy will help kids and young people to feel more confident communicating with others.
Speech pathology therapy can also assist kids and young people with disability experiencing difficulties with communicating, eating and drinking.
Midel group Speech pathologist can also help you with
- Developing social skills
- Voice and resonance, including loudness of voice and pitch quality.
- Cognitive Communication including social communication skills, problem-solving and reasoning skills.
- Learning how to read and write.
- Developing talking skills and fluency
Midel group Speech pathologist will also work with participants, parents, and other support person to provide information, guidance, resources and develop an intervention program that best meets the communication needs of the participant with disability.
